The Medical Scribe at Oak Street Health supports primary care providers by managing clinical documentation, enabling providers to focus on patient care. Responsibilities include participating in daily huddles, observing and documenting patient encounters in real-time, and following up on next steps post-visit. The role aims to improve the accuracy, specificity, and timeliness of clinical documentation. Referred internally as Clinic Informatics Specialists (CISs), scribes help close care gaps and enhance patient outcomes through expertise in value-based care and chronic condition management, including ICD-10 and CPT coding. They receive extensive on-the-job training in clinical workflows, value-based medicine, and preventative care, preparing them to support providers effectively and improve documentation practices. The ideal candidate is committed to at least 1-2 years in the role, making it suitable for individuals on a pre-med track or pursuing careers in Health Informatics, Public Health, Healthcare Administration, Medical Coding, and related fields. This paid, practical experience in a clinical setting is valuable for career advancement. Key responsibilities include: Document patient encounters (~80%), including history, assessment, plan, physical exam, and coding. Join providers in exam rooms to observe visits and document in structured notes. Prepare after-visit summaries and consult with providers for documentation accuracy. Support clinical documentation improvement (~10%) by reviewing medical records and leveraging population health tools.
